The ECHO Foundation is thrilled to sponsor a golf tournament this September. Proceeds will support the ECHO Foundation which is a unique, non-profit organization dedicated to providing support services, guidance, and education to cancer patients and their caregivers.

Participation includes possible exposure to and illness from infectious disease including but not limited to COVID-19. While participation rules and personal discipline may reduce risk, the risk of serious illness and death does exist; and,
I KNOWLINGLY AND FREELY ASSUME ALL SUCH RISKS both known and unknown and assume full responsibility for my participation and,
I willingly agree to comply with the stated and customary terms and conditions for participation as regards protected against infectious disease. If, however, I observe any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest event official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin HEREBY RELEASE AND HOLD HARMLESS the ECHO Cancer Foundation, their officers, Board of Directors, other participants, sponsors, and volunteers used to conduct the event. WITH RESPECT TO ANY AND ALL ILLNESS, INJURY, DISABILITY, DEATH or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEE OR OTHERWISE, to the fullest extent permitted by law.
