Our Patient Portal: Navigating Care
ECHO offers a useful online patient portal called Navigating Care. This portal allows you to manage your personal medical records and get the support you need with 24/7 secure online access from any web-enabled device.

Navigating Care allows you to perform a number of useful tasks:
- Access your personal health information 24/7
- Receive personalized health education
- Prepare for upcoming doctor visits
- View lab and test results
- Securely communicate with your care team
- Make appointment requests online (non-urgent)
To Join:
Joining is simple. Please provide your full name, email address, and date of birth to a staff member at check-in. Or, complete the Navigating Care card at the check-in area. Once we have your email, look out for an email invite to get started. You will need to go to patient.navigatingcare.com and follow a few simple steps. We must have your email in order to get you set-up on the portal. If you don’t have an email to provide, we can give you temporary credentials.
